Returning Students

Thank you for choosing Pomelo - again!

The following items must be submitted yearly and will be sent home the first week of school:

• Student Emergency Information Form
• Student Housing Questionnaire (if nothing applies, please complete top portion, check NO, sign bottom and return)
• Publicity Authorization and Release (Required yearly even if declining)
• Migrant Education Family Questionnaire (Only if changed from original enrollment document)
• American Indian-Alaskan Native Letter Questionnaire (Only if changed from original enrollment document)
• Title III Form (Only if Applicable)
• Responsible Use Policy (All 3 pages) (Parent initials each line and signs last page, student initials/signature optional)
